Liability Waiver and Release of Responsibility Agreement

Important for Non-Members of the NPSF (Dutch Pencak Silat Federation)

Are you training at Warisan International Martial Arts Academy but do not wish to become a member of the NPSF?
Then you are required to truthfully complete and sign the Waiver / Liability Declaration in duplicate.

• One copy is for our records.

• One copy is for you to keep.

Without this signed declaration or a valid membership with the NPSF, we unfortunately cannot allow you to participate in training.
